SS-31 (elamipretide, MTP-131) is a mitochondria-targeting tetrapeptide that binds selectively to cardiolipin on the inner mitochondrial membrane. Cardiolipin binding stabilises electron-transport-chain supercomplexes and reduces mitochondrial ROS leakage. This mechanism places SS-31 in a different pharmacological class from most other peptide references — it acts at the organelle level rather than through a plasma-membrane receptor.

Clinical development (under the name elamipretide) has included Barth syndrome, primary mitochondrial myopathy, geographic atrophy in age-related macular degeneration, and heart failure. Some programmes have advanced, some have failed key endpoints — a nuanced evidence base that makes SS-31 a valuable reference compound for mitochondrial-bioenergetics research.
